Case Management Services
Case Management is a service that assists children, adolescents and their families with mental illnesses or emotional disorders by gaining access to psychiatric, medical, social, educational, and other services. The level or type of case management service is determined by the intensity of the needs of the child, adolescent and family. Case management services include linking with services, monitoring of service delivery, gaining access to services, assessment and service planning, problem resolution and help in using community resources.
Population Served: Service is available to children, adolescents and families who need assistance in accessing services in particular the behavioral health system. Special care is taken for cases where children have had numerous placements including hospitalizations.
Services include: There are three levels of Case Management services; Intensive Case Management, Resource Coordination and Blended Case Management. Each child or adolescent, if eligible, is assigned an individual case manager who will work with the child and family to monitor and assess services. The type of case manager will depend on the level of need. Children, adolescents and families could be seen from every two weeks to once a month depending on level of care needed.
